Since the end of our spring Continuous Care Systems (CCS) cycle, the navigation leads have been working diligently to coordinate clients from Center of Hope and UT Southwestern student navigator teams for a summer cycle. The summer CCS cycle is expected to last for 7 weeks, and three teams with 9 navigators total have been formed to work with 3 new clients! For our Standardized Care Systems program (SCS), the cycle will be a similar length and is projected to also have 3 teams which will serve 3 clients at the Calvert Men’s Shelter. As the summer cycles have just begun, PNP is excited to report the teams’ progress once the clients’ goals have been established and addressed!
