Our fellows have concluded the first cycle of the Continuous Care Navigation System (CCS) and it has been such a success. We have reached so many SMART goals with our clients and assisted them in accessing the medical care that they have been seeking. We are grateful for the support of all of our partners who have worked so hard to connect our clients with their resources. We look forward to learning from this experience to improve during our second cycle which will be starting shortly.
