Now that we’ve gone through two months of the Continuous Care Navigation System (CCS) we have begun to pilot our other program, the Standardized Care System (SCS) with 3 clients. Our pilot team will work with these clients to establish and meet their health goals and learn how to make this system sustainable for the future. We can’t wait to see what our team is able to achieve with our client. Once we pilot the SCS we can start to see how the two programs will work together.
