The first month of our Continuous Care Navigation System (CCS ) is now complete and we couldn’t be happier with how it has gone. We expanded our launch to 9 clients instead of the 8 that we anticipated. Our teams of navigators are using all of the skills that they have learned to set SMART goals for their clients. Our navigators spent a whole semester preparing for this launch, and we are so proud of the hard work that they have put into helping our clients reach their goals.
