Happy summer everyone!
With our first 15-week cycle of the Continuous Care system completed, the PNP team is extremely proud of our navigation teams and all they have accomplished with their clients! They successfully served 6 clients at the Center of Hope Shelter and the teams who spent the full cycle with their clients had several significant achievements! We are excited to start our summer cycle in the coming week or two and are looking forward to reporting those teams' progress soon.
