Our first cycle of the Continuous Care Navigation System (CCS) is coming to a close and we have begun piloting our Standardized Care System (SCS) for more acute needs. We are so happy with the progress that we have made with both programs. We have so many potential participants who are interested in our programs and we look forward to serving them all. The excitement that we have seen from these individuals tells us that we are succeeding in filling the need that we saw.
